Office Information
When you arrive at the Mystwood Professional Building, you are welcome to park anywhere in the parking lot, with the exception of the fire station. The address is on our building as well as a sign with a list of names (mine included) next to the building entrance, which will bring you into the waiting room. You will need a door code to enter. This will be provided in your new patient email.
Once inside the waiting room, there is a panel of buttons with each practitioner’s name. Press the one under my name, just once, and it will let me know you have arrived. There is no front office staff, so there will not be a person to greet you.
Insurance Information
The State of California allows direct access to physical therapy services, but if you are choosing to bill your insurance company, we require a doctor’s prescription.
Due to the challenges of working with insurance companies, Hands-On Physical Therapy has made a transition to a more cash-based practice. We anticipate this change will be beneficial as it will free us up from working within the complex and restrictive nature of insurance companies and allow us to concentrate all of our efforts on therapy.
We will continue to work with and provide direct billing for patients with Humboldt IPA, Medicare, TriWest, and some worker’s compensation companies.
We are out of Network with Anthem Blue Cross, Blue Shield, Aetna, Humana, GEHA, United Health Care, Cigna, and Tricare. For patient’s with these insurances we can provide a super bill for people who choose to seek out reimbursement on their own.
We do not take Partnership/Medi-Cal, Medicare with Partnership as a secondary, Medicare Advantage Plan, or any motor vehicle clients.
Work Comp insurance companies will need to provide a signed Request for Authorization (RFA) with adjustor’s phone number and email, doctor’s chart notes, the authorization letter (from the primary insurance not just the third-party insurance), and an expiration date before we will review the case and schedule the injured worker. Please take note that if all these documents, that we require, are not provided up front we will not be able to schedule the injured worker.

Cancellation Policy
As a way to maintain a respectful client and therapist relationship and to value everyone’s time, we charge for missed appointments and late cancellations/rescheduling. In order to optimally serve all of our clients’ needs we require 2 full business days for any rescheduling. If we receive less than 2 business days notice there will be a $50 fee. If no notice is given and you miss the appointment, the full appointment fee will be charged. It you have an insurance that does not allow for us to bill for missed appointments, you may be discharged from the practice after the first late cancellation or missed visit.
